The special medicine has the same properties as before, and is again made by mixing two strong medicines in the [[Krak Pot]]. Its sale price has dropped considerably down to a mere 35 gold coins since alchemy works instantly in this game. It can also be refined further into a [[panacea]]. Special medicine can also be dropped by [[badja]]s and [[great troll]]s.

Special medicine is an exceptional healing item in the Dragon Quest series. The special medicine usually restores at least 90 or more HP to a single party member. In games where alchemy appears, it can be created by combining two strong medicines together.

Making their debut, the special medicine will restores 90~120 HP to a single party member. It is created by mixing two strong medicines in an Alchemy Pot. Three special medicines can be found in treasure chests in the Maella Region, Empycchu, and at Marta's Cottage after acquiring the ultimate key. They can also be commonly dropped by jabberwockees and boss trolls. Special medicine is also the first item that Dodgy Dave in Pickham will request, giving the player 500 gold coins in exchange for the first time, then 400 for subsequent turn-ins. In the Nintendo 3DS version, special medicine can also be found randomly in level 2 blue treasure chests.

In the original Sony PlayStation 2 version, special medicine can be sold for 170 gold coins each for the first 29 sold, 510 gold each for the 30th to 49th sold, then back to 170 for the 50th onward. Since the Alchemy Pot works instantly in the Nintendo 3DS version, special medicine can only be sold for 96 gold coins each for the first 29 sold, 64 gold each for the 30th to 49th sold, then only 32 from the 50th onward.

Special medicine can also be used to make more potent restorative items in the Alchemy Pot, as well:

In this game, special medicine can be purchased from various item shops and roving emporiums around the world for 70 gold coins each in the postgame and sold for 35 gold. Special medicine can also be found in some barrels at the Last Bastion, Heliodor Castle, and Dundrasil Castle during the second act. Some of the soldiers at Heliodor Region will also give the player special medicine as a gift for defeating monsters before facing the Headless Honcho. In battle, malicious needlers and electric needlers can use special medicine to heal themselves or one of their fellow monsters.

Special medicine can also be dropped by vicious stump chumps, vicious bongo drongos, vicious orcs, vicious stump grumps, tentaculars and their malicious counterparts, malicious needlers, electric needlers, malicious she-slimes, and malicious muddy hands.

Special medicine will restore around 100~120 HP to whichever character the player is controlling. They can be dropped by some monsters and found in smashed pots and barrels in higher levels of the game. Like other healing items, they do not carry over between stages and should be used as needed.
